NSW Ambulance paramedics were called to the crash site near Nowendoc about 3.20am this morning.
A spokesman for the Hunter region's Westpac Rescue Helicopter service said the woman was a passenger in the Pantec truck which rolled off the roadway down an embankment at about midnight.
The driver was uninjured, but it took about four hours before he was able to flag down a passing motorist and alert emergency crews to the accident.
The woman was treated at the scene by paramedics and transported to Gloucester Soldiers Memorial suffering a cut to the head and facial injuries.
She was later flown in a serious condition to John Hunter Hospital.
